Healthy Environments Active Living Program (HEAL)
The Utah Department of Health and Human Services, Healthy Environments Active Living Program (HEAL) is seeking applications to fund up to two Community Based Organizations. The purpose of this funding is to reduce disparities related to chronic disease, specifically diabetes and prediabetes, by addressing the gaps in the social determinants of health within priority populations.
The agreements resulting from this Request for Grant Applications (RGA) will be for a period of five years, 2023-2028. Funding will be renewable by contract amendment each year depending on project performance and available funds. Project years will run as follows:
Year 1: 3/1/23 to 6/30/24
Year 2: 7/1/24 to 6/30/25
Year 3: 7/1/25 to 6/30/26
Year 4: 7/1/26 to 6/30/27
Year 5: 7/1/27 to 6/30/28
Funding for this announcement comes from a mix of state and federal funds. Applicants may apply for up to $75,000 the first year and $30,000 per year for the remainder of the contract.
Applications are due to Karlee Walker at [email protected] on Friday, January 19, 2024 by 11:59pm. Any applications submitted after this time will be disqualified.
Q&A Sessions: One informational call will be held for potential grant applicants. This session will give applicants an opportunity to learn more about the grant, HEAL, ask questions, and receive necessary guidance. Conference call information will be provided to applicants once they have RSVP’d to Karlee Walker by December 11, 2023. This call will be recorded and can be shared with those who are unable to attend. The date for Q&A call is as follows:
December 13, 2023: 11 AM – 12 PM

Applicants should review Appendix F for useful state, community, and national organization websites, utilize existing data and help complete the applications.
